It's not unique that the guitar and voice are the first two instruments (such as in "Drain You"), but it is rarer for the voice and guitar to start at the same time. Low does not make it seem rare, because they have several:
"Immune," "Don't Carry It All," "The Plan," "Same," "One Special Gift," and "Hey Chicago."
"Over the Ocean" was already mentioned.
The only other un-mentioned one I can think of is Pearl Jam's "Elderly Woman Behind the Counter at a Small Town."
Jesus Lizard gets a bonus for starting an album this way.
