iPhone users: help me source a photo app please...

I can't go into tons of detail but my job involves spending public money on alternative solutions for delivering health and social care services. One such service is the ability for health conditions to be diagnosed remotely by someone taking a series of photos and them being sent for diagnosis as opposed to the actual person being sent for diagnosis.I'm tearing my hair out trying to find a solution to a particular problem I'm facing with this and, as a non-Apple devotee, I'm lacking experience in the various apps available to be able to do this. What I'm after is:iPhone app that allows the user to pre-name and pre-size the photos they're about to take. So, you choose a name that then applies to each photo you then take (differentiated by a number added to the end: i.e. you pick "Simmo bums a dog" as the name and then you take 3 images and it names them "Simmo bums a dog 1" to "Simmo bums a dog 3"). You can then compress the standard iPhone image size to whatever you want (either MB size or pixel size - whatever). You then use the camera, the app does it's renaming and resizing magic and you can then access the files created and drag them across to your own Desktop computer via USB connection.Restrictions - can't have ads on it and can't have the risk of any kind of spyware/adware/advertising pop-ups occurring at all. It has to be totally legit and locked down to do what it's supposed to do.It also can't rely on Internet connection in anyway - I say this because Airwatch Content does what I need it to but the app stores the photos inaccessibly as it's all about transferring the files to cloud-type storage via internet connection and that's a no no.Someone must have encountered this problem before?
