One way it can work is adopting ridiculous names as a joke, such as "Sally Pumpkin Button from Beyond the Stars" or "Hector Berlioz's Third Cousin Jed" .

Ex:

"Oh Sally Pumpkin Button from Beyond the Stars, would you refill my coffee cup while you're up?" "Certainly Hector Berlioz's Third Cousin Jed, right after I feed the cat."

If you make this joke consistently enough, you start doing it without thinking, and eventually you're just calling each other Sally and Hector, then Sal and Heck, and it's your own private joke and a genuine term of endearment.

Either that or she says that you should be able to be affectionate without making some big joke out of it... ... and you have to refill your own coffee cup.
