I was always taught to never shake a woman's hand unless she offers it to be shaken (shook?). (i.e. the man doesn't offer his first)
Following this rule can make for some ackward introductions.

I think this rule is obsolete. If I were hobnobbing with British nobility, I might wait for hands to be offered before I reached out. But in neo-USA 20x6, I'll meet a woman like I meet a man, with my hand held out. Nice to meet you.

I was just watching a TV show about etiquette. Apprently, the custom of shaking hands originally served to demonstrate that neither party was carrying a weapon.
Better safe than sorry, I guess.
